*1 Use these forms if you are adding (or replacing existing data with) data that were obtained under the same policy as the one used for already submitted data (i.e., data that were obtained with the same consent form and that were subject to the same ethics review approval). Submit a new application for data submission if the data policy is different.
*2 Should your affiliated institution change, contact us at your earliest convenience and complete the necessary procedures in a timely manner because continuation of the study requires ethics review for the study at the new institution and submission of an application for data use (for the new institution). We appreciate your cooperation.
In creating the NBDC Guidelines for Human Data Sharing and the NBDC Security Guidelines for Human Data, the NBDC Steering Committee set up the NBDC Data-Sharing Subcommittee in order to examine various issues.
